The highest point of East Cairn Hill is flat-topped and featureless but the 561m North summit (NT 122596) is a fine viewpoint with steep ground in most directions.

The easiest approach to the summit is from the Bore Stane (NT 144599), reached by a long but pleasant track from Carlops. The route to the summit via The Mount and Wether Law is rough with steep short climbs between these points, leading to a demanding Carlops-based round. An even longer round from Baddinsgill via Mount Maw, The Mount, West Cairn Hill and Byrehope Mount can be made by including West Cairn Hill's fine SW ridge, although finding a remotely pleasant route between the foot of this ridge and the end of the the drove road may not be possible.
